Membership Categories and Application Requirements

Voting Members

1. Hold a Doctorate degree (M.D., D.O. or Ph.D. in a Life Science).
   • Have an interest in the care of patients with Lyme and associated diseases.
   • Involved in research related to Lyme and associated diseases.
2. Non-voting member who is elected to be voting by the Board of Directors.

Voting members may serve on the Board of Directors, hold elective offices and participate at all levels of committees.

• Application Requirements for Voting members:

• Completed application
• Copy of current license
• Copy of curriculum vitae
• Sponsorship of two voting members
• Statement of interest including information about applicant's interest in Lyme and associated diseases, what applicant hopes to gain from membership and what applicant hopes to offer ILADS
• Payment of $250 for first year's dues to accompany application

Non-Voting Members

1. Affiliate Members: Allied healthcare professionals with an interest in the diagnosis, treatment, management and/or research of Lyme and associated diseases.
2. Student Members: Medical students who are enrolled in full-time healthcare training program and have an interest in learning about Lyme and associated diseases.
3. Retired Members: Those with Doctorate degree (M.D., D.O. or Ph.D. in a Life Science) who are retired from medical practice and/or research related to Lyme and associated diseases.

• Application Requirements for Affiliate and Retired Members:

• Completed application
• Copy of current license
• Copy of curriculum vitae
• Sponsorship of two voting members
• Statement of interest including information about applicant's interest in Lyme and associated diseases, what applicant hopes to gain from membership and what applicant hopes to offer ILADS
• Payment of $175 for annual dues to accompany application

• Application Requirements for Students:

• Completed application
• Copy of curriculum vitae
• Sponsorship of one voting member
• Statement of interest including information about applicant's interest in Lyme and associated diseases, what applicant hopes to gain from membership and what applicant hopes to offer ILADS
• Verification of enrollment in a healthcare related training program. Application signed by residency/intern program chairperson or graduate school.
• Payment of $100 for annual dues to accompany application

Special Status Within Corporation

1. Fellows — determined by the Board of Directors
2. Honorary Member
   • Persons who have made and exceptional or distinguished contribution and have an interest in the well being of ILADS and its service to the medical community and the public.
   • Non-voting and do not serve on committees
   • Conferred by a unanimous vote of the Board of Directors.
   • Name must be submitted to the Board of Directors by four voting members
3. Sustaining Member
   • Persons ineligible for medical categories of membership, who participate in a Lyme and associated diseases related industry and have an interest in the well being of ILADS and its service to the medical community and the public.
   • Non-voting and do not serve on committees
   • Conferred by a unanimous vote of the Board of Directors

• Application Requirements for Sustaining Members:

• Completed application
• Curriculum vitae
• Sponsorship of two voting member
• Statement of interest including information about applicant's interest in Lyme and associated diseases, what applicant hopes to gain from membership and what applicant hopes to offer ILADS
• Payment of $175 for annual dues to accompany application
